Required Administrative/Eligibility Forms and Documents
In order to participate in athletics at Lackawanna College, the following forms must be completed and returned to the Lackawanna College Athletic Department. You must complete both the administrative and athletic trainers forms found on a seperate link under the Inside Athletics tab.
If these forms are not completed and returned to the Athletic Department, you will not be able to practice or play until they are finished and delivered to that office.
Therefore, please complete and mail the finished forms to:

IMPORTANT, REQUIRED DOCUMENTS!
The following documents must be submitted to the Lackawanna College Athletic Department prior to participation in any Lackawanna College Sponsored Athletic Event. You will not be able to practice or play until these documents are received by the Athletic Department.
Medical Physicals and Insurance:
All student/athletes participating in athletics at Lackawanna College must have a physical clearing them to participate in their sport ( link to athletic trainers forms and information ) and have their own medical insurance coverage. They must submit a copy of their medical insurance card to the athletic trainers before participating in any practices or games in their respective sport.
Proof of High School Graduation or GED:
As part of its eligibility rules, the NJCAA requires its member institutions to obtain proof that its student athletes have graduated high school, or in the alternative, have obtained a General Education Diploma. Therefore, all student athletes participating in athletics at Lackawanna College must provide the Athletic Department with an official copy of their high school transcript showing that the student athlete has graduated from high school. The transcript must include the:
* name of high school;
* name of the Student;
* date of graduation
* signature of a school official
Or, in the alternative, a copy of the paperwork evidencing that the student athlete has successfully obtained a General Education Diploma. A copy of official high school transcript showing the number of years attended while a high school student must also be submitted.
In addition to the requirement to provide proof of High School Graduation or a General Education Diploma, a transfer students must also provide to the Lackawanna College Athletic Department a copy of an official transcript from each college he or she has attended. If a student has not withdrawn from that college within 15 days of the beginning date of the semester/term, a copy of the original transcript will be required whether or not the student has completed the semester/term. If the student has withdrawn within 15 days of the beginning of the semester/term, the student shall provide to the Athletic Department written proof of their withdrawal showing both the starting date of the semester and the date of withdrawal.
The student/athlete will not be eligible to participate in any collegiate games, contests, or events until the required transcripts have been received and filed in their eligibility file.
