Required Pre-Participation Forms and Requirements

In order to participate in athletics at Lackawanna College, the following forms must be completed and returned to the Lackawanna College Athletic Department. You must complete both the administrative and athletic training forms.

IMPORTANT!

If these forms are not completed and returned to the Athletic Department, you will not be able to practice or play until they are finished and delivered to that office.

It is recommended that you hand deliver the completed forms to the Athletic Department on your arrival at Lackawanna College. The department's offices are located on the second floor of the Lackawanna College Student Union, 500 Jefferson Avenue, Scranton, Pennsylvania.

Warning: If you do mail the completed forms make sure that they are sent well in advance or your scheduled arrival on campus. You will not be able to practice or play until they are received by the Athletic Department.

Athletic Training Forms

Proof of High School Graduation or GED:

As part of its eligibility rules, the NJCAA requires its member institutions to obtain proof that its student athletes have graduated high school with a academic diploma, or in the alternative, have obtained a General Education diploma. Therefore, all student athletes participating in athletics at Lackawanna College must provide the Athletic Department with a copy of their high school diploma; or, a high school transcript showing that the student athlete has received a high school diploma and the date on which it was received; or, a copy of the their General Education diploma.

Transfer Students:

In addition to the requirement to provide proof of High School Graduation or a General Education diploma, transfer students must also provide to the Lackawanna College Athletic Department a copy of a transcript from each college he or she has attended. If a student has not withdrawn from that college within 15 days of the beginning date of the semester/term, a copy of a transcript will be required whether or not the student has completed the semester/term. If the student has withdrawn within 15 days of the beginning of the semester/term, the student shall provide to the Athletic Department written proof of their withdrawal showing both the starting date of the semester and the date of withdrawal.
